- BMI that is no greater than 36
- Is not bedridden, confined to a hospital, nursing home, long-term care facility, skilled nursing facility nor advised to receive care in one of these facilities, in hospice care or through home health care.
- Is not confined to a wheelchair, electric scooter due to a chronic illness or disease that requires assistance with activities of daily living
- Does not require oxygen equipment to assist with breathing
- Has not been diagnosed with diabetes nor have any related complications including retinopathy, nephropathy, neuropathy, or peripheral vascular disease (PVD or PAD), or ever had insulin shock or been in a diabetic coma
- Never been diagnosed with AIDS, ARC, HIV, Alzheimer’s, dementia, Huntington’s disease, sickle cell anemia, myelodysplastic syndrome (MDS), Lou Gehrig’s Disease (ALS), Quadriplegia, Paraplegia, Down’s Syndrome, mental incapacity, congestive heart failure, cirrhosis, metastatic cancer, recurrent cancer of the same type, Hepatitis C, chronic lung disease, including chronic obstructive pulmonary disease (COPD), chronic bronchitis, emphysema, and sarcoidosis, or prior to age 25, cerebral palsy, spina bifida, cystic fibrosis, or muscular dystrophy
In the past 2 years:
- No treatment or advisory to receive treatment for coronary artery disease, heart attack, coronary artery bypass surgery, angioplasty, cardiomyopathy, irregular heart rhythm, or valvular heart disease with surgical repair or replacement, stroke or transient ischemic attack (TIA)
- Has not been hospitalized for any mental or nervous disorder
In the past 4 years:
- Has not received treatment or advised to receive treatment for cancer, leukemia, melanoma, or any other internal cancer (except basal or squamous cell skin cancer), chronic kidney disease, systemic lupus, scleroderma, bipolar depression, schizophrenia, Parkinson’s disease, or multiple sclerosis
In the past 5 years:
- Been convicted more than once of reckless driving or driving under the influence of drugs or alcohol
In the past 10 years:
- Not been convicted of or currently awaiting trial for a felony
- Been treated for or advised to have treatment for alcohol or drug abuse

- Never been diagnosed with AIDS, ARC, HIV, Alzheimer’s, dementia, Huntington’s disease, sickle cell anemia, myelodysplastic syndrome (MDS), Lou Gehrig’s Disease (ALS), Quadriplegia, Paraplegia, Down’s Syndrome, mental incapacity, congestive heart failure, cirrhosis, metastatic cancer, recurrent cancer of the same type, Hepatitis C, chronic lung disease, including chronic obstructive pulmonary disease (COPD), chronic bronchitis, emphysema, and sarcoidosis, or prior to age 25, cerebral palsy, spina bifida, cystic fibrosis, or muscular dystrophy

- Have been diagnosed with AIDS, ARC, HIV, Alzheimer’s, dementia, Huntington’s disease, sickle cell anemia, myelodysplastic syndrome (MDS), Lou Gehrig’s Disease (ALS), Quadriplegia, Paraplegia, Down’s Syndrome, mental incapacity, congestive heart failure, cirrhosis, metastatic cancer, recurrent cancer of the same type, or prior to age 25, cerebral palsy, spina bifida, cystic fibrosis, or muscular dystrophy
- Alternative: Guaranteed Acceptance Plan (if meets age requirements: Issue Ages 40-80)
